Does Teeth Whitening Damage Enamel?

Short answer
Research has not shown lasting enamel damage from peroxide whitening used as directed. The side effects that are well documented are temporary tooth sensitivity and gum irritation. Long-term studies of enamel are limited, so the accurate summary is "no damage shown", not "proven harmless".
What does the research say about enamel?
The largest review of whitening at home is a Cochrane review of 71 trials covering 3,780 adults. It found the most common side effects were tooth sensitivity and irritation of the mouth, that both were mild and temporary, and that both were more common with stronger gels. Its summary does not report enamel damage.
The American Dental Association notes that some research suggests whitening may change the roughness of the enamel surface. It does not describe this as lasting damage.
Why do some studies say whitening softens enamel?
Many of those studies were done on teeth in a laboratory, not in a mouth. A 2016 study in the Brazilian Dental Journal tested both ways with 10% hydrogen peroxide. Enamel in the laboratory lost hardness. Enamel worn in volunteers' mouths did not. The authors credit saliva, which restores minerals to the tooth surface.
That study was small, 60 pieces of enamel from 30 teeth, and it tested one strength.
Does a stronger gel do more harm?
Stronger gels cause more sensitivity and gum irritation, according to the Cochrane review. None of the sources reviewed for this article showed enamel thinning or wearing away at any strength.
Stronger products are made to be worn for less time. Opalescence Go 15%, for example, is worn for 15 to 20 minutes a day. Keeping to the wear time on your product is the simplest way to stay inside what has been tested.
Can you whiten too often?
This is where the evidence runs out. The trials in the Cochrane review lasted from two weeks to six months, so they do not show what years of frequent whitening does.
Who should not whiten?
The NHS says whitening is not recommended if you are under 18, pregnant or breastfeeding, or if your teeth or gums are unhealthy. The American Dental Association adds that only natural teeth whiten, so crowns and fillings will not change colour.
